Brief CBT moves the strands of suicidal thinking faster and together

When monitoring suicidal ideation, watch its separate parts, because an improving total can hide a wish to die that has not moved.

Brief cognitive behavioural therapy (BCBT) is known to prevent suicide attempts better than present-centred therapy, though both lower overall suicidal ideation scores to a similar degree. This secondary analysis of a randomised trial in high-risk US military personnel and veterans tracked five separate items of the Scale for Suicidal Ideation at every session: wish to live, wish to die, reasons for living against dying, suicidal desire and passive wish for death.

In both treatments some dimensions changed quickly and others slowly. Under BCBT the dimensions moved faster and more in step with each other than under present-centred therapy.

The analysis is mechanistic and the paper reports patterns rather than a single effect size, so it adds explanation rather than a new reason to choose BCBT. Its practical point is that a falling total score can hide a dimension, such as the wish to die, that has not moved, and that tracking the parts separately is informative.

  • Track wish to live and wish to die separately rather than relying on one total suicidality score
  • Look for a dimension that has not shifted even when the total score improves
  • Consider a structured suicide-specific therapy such as BCBT where it is available
  • Keep safety planning in place while scores are still changing

Why it matters

It suggests why a suicide-specific therapy prevents attempts better even when overall ideation scores look the same.

Don't overread it

This is a secondary, mechanistic analysis in a military population; it does not add new evidence on attempt prevention.
