Lithium prophylaxis was linked to half the postpartum recurrence rate in bipolar disorder

A pooled look at lithium around delivery, a cognitive-remediation trial that found personalising the exercises added nothing, a mindfulness programme for opioid use disorder, and a US buprenorphine injection recall.

The edition in brief

Across eight observational studies of 554 women with bipolar disorder, postpartum recurrence was 21% (95% CI 15-29%) in those taking lithium prophylaxis and 41% (33-50%) in those who were not. Confounding by indication limits how far that can be read as a treatment effect, but it supports planning prophylaxis before delivery rather than after the first relapse. In a randomised trial of 184 outpatients with schizophrenia, adding early auditory processing training to cognitive remediation did not improve cognitive or functional outcomes, even in those with auditory deficits; cognitive remediation itself produced moderate gains across both arms. A small fMRI trial of Mindfulness-Oriented Recovery Enhancement in inpatients with opioid use disorder found changes in prefrontal responses to drug cues and longer abstinence at about four months than supportive therapy, with a small effect size in a subset of 38. In the US, a Class II recall covers one buprenorphine hydrochloride 0.3 mg/mL injection for crystallisation.
