Gadolinium-based contrast agents remain safe, but regulators have withdrawn several linear agents and the standing advice is the lowest effective dose. Many follow-up brain MRIs are booked with contrast by default when the clinical question does not require it.
Stable multiple sclerosis follow-up, headache without red flags and some post-treatment surveillance can often be answered without contrast. Where contrast is needed, use a macrocyclic agent at the lowest diagnostic dose.
