If rectal gas distorts the diffusion sequence or motion blurs the T2 images, the reader of a PI-RADS 3 has no way of knowing whether the equivocal call reflects the gland or the scan. Say so in the report: name the degraded sequence and whether it limits assessment. A short line such as "DWI degraded by rectal gas; peripheral zone assessment limited" tells the urologist whether to trust the score, repeat the scan or proceed to biopsy.
- Name any degraded sequence in the prostate MRI report.
- State whether image quality limits the PI-RADS assessment.
- Suggest a repeat scan when quality prevents a confident score.
