This population study from the Study of Health in Pomerania derived reference values from 1,037 German men without prostate cancer, benign prostatic hyperplasia or urinary symptoms, with 10 years of follow-up. Prostate volume was measured on MRI by deep-learning segmentation.

Median prostate volume was 31.3 mL (5th to 95th percentile 20.7 to 51.4), PSA 0.81 ng/mL (0.32 to 2.79) and PSA density 0.025 (0.011 to 0.071). PSA and volume rose steadily with age, but PSA density was more stable, peaking at 71 and plateauing to 75. Age-adjusted PSA density never exceeded 0.11. Body mass index was associated with larger prostates and lower PSA density. An open-access calculator accompanies the paper.

The practical message is that PSA density does not drift upward with age the way PSA does, which supports using fixed PSA density thresholds (commonly 0.15 or 0.20) across age groups. The cohort was young (median 46) and German, so the upper age percentiles rest on fewer men and may not transfer to other populations.

• Report prostate volume and PSA density with every prostate MRI.
• Treat a PSA density above 0.11 as outside the range seen in healthy men of any age in this cohort.
• Do not raise PSA density thresholds for older men; density did not climb with age as PSA does.
• Account for body mass index: heavier men had larger glands and lower PSA density.

Why it matters

It gives a normal range for a number increasingly used to decide who needs biopsy.

A young, German population; reference values for older men and other ethnic groups may differ.
