Most patients with stable kidney function can receive intravenous iodinated contrast without special precautions. The patients worth identifying are those with a markedly reduced eGFR, acute kidney injury or unstable renal function.
For metformin, guidance from major radiology bodies limits withholding to patients with acute kidney injury or severe chronic kidney disease, not every patient on the drug.
- Check a recent eGFR in patients with known kidney disease or acute illness
- Flag acute kidney injury before contrast, as risk is concentrated there
- Withhold metformin only where kidney function is severely reduced or acutely worsening, per local policy
- Do not delay urgent contrast imaging for a creatinine result in an emergency
Why it matters
Over-cautious contrast rules delay diagnosis for many patients to protect very few.
