Treat-to-target depends on a number, yet many notes record 'improving' or 'better on JAKi'. A change-based impression is exactly the measure that predicted least in today's registry.
Record tender and swollen joint counts, patient and physician global scores and the CDAI total at each visit. It takes two minutes and makes the week-12 decision straightforward.
- CDAI = tender (28) + swollen (28) + patient global (0–10) + physician global (0–10).
- Remission ≤2.8; low activity ≤10.
- Write the number, not 'improving'.
