In lupus, glucocorticoid exposure over the past one to three years carried the organ damage risk
A Swedish nationwide cohort of 3,749 patients links recent oral glucocorticoid dosing to first organ damage. Also: two-year spinal MRI results for bimekizumab in radiographic axial spondyloarthritis, antimalarials and kidney biopsy findings in lupus nephritis, and a feasibility trial of anakinra for gout flares in chronic kidney disease.
The edition in brief
A Swedish register study of 3,749 patients with newly diagnosed lupus found that the increased risk of first organ damage with oral glucocorticoids was well captured by dosing over the previous two years (adjusted HR 1.42 per 5 mg/day, 95% CI 1.31 to 1.53) and fairly well by the previous year (1.34), while cardiovascular damage depended on dose over the past three years. A phase 3 MRI sub-analysis of 102 patients with radiographic axial spondyloarthritis on bimekizumab showed lower spinal inflammation sustained to week 104 with minimal change in erosions and new bone. In 165 patients with lupus nephritis, antimalarial use was not associated with differences in biopsy activity, chronicity or fibrosis scores. A feasibility trial of anakinra versus intramuscular methylprednisolone in 21 patients with gout and kidney disease missed its recruitment target and cannot show efficacy.
Bimekizumab on spinal MRI: inflammation fell and stayed low for two years
Bimekizumab reduced spinal MRI inflammation durably in this analysis, but structural benefit is unproven.
Antimalarial use and kidney biopsy findings in lupus nephritis: no detectable difference
Continue antimalarials in lupus as per current guidance; this small biopsy study does not argue otherwise.
Anakinra for gout flares in kidney disease: a feasibility trial that missed its recruitment target
Do not change gout flare treatment in kidney disease on this feasibility study.
Record the glucocorticoid dose and the plan to reduce it at every lupus visit
Document steroid dose and a dated reduction target at each lupus visit.
In lupus, review glucocorticoid exposure over the last one to three years, not just the current dose
Consider reviewing cumulative steroid exposure over the past one to three years when assessing damage risk in lupus.
