Belimumab in lupus nephritis: deeper renal responses took two years to build

Allow up to two years to judge depth of renal response on add-on belimumab in lupus nephritis.

Design
Retrospective single-centre cohort with paired-biopsy subgroup
Population
43 patients with biopsy-proven lupus nephritis not in complete response, on add-on belimumab
Primary outcome
Deep clinical renal response (uPCR <200 mg/g, no active haematuria, stable function)
Effect
38.5% at 12 months, 53.8% at 24 months (vs CRR 66.7% and 76.9%)

Standard complete renal response in lupus nephritis allows a urine protein-to-creatinine ratio up to 500 mg/g, more residual proteinuria than is accepted in other glomerular diseases. A single-centre cohort in Rheumatology studied 51 patients with biopsy-proven lupus nephritis given add-on belimumab between 2015 and 2025, analysing 43 not already in complete response.

A stringent 'deep' response — protein-to-creatinine ratio under 200 mg/g, no active glomerular haematuria and stable renal function — was reached by 38.5% at 12 months and 53.8% at 24 months, against 66.7% and 76.9% for conventional complete response. Deep responses came later (median 17.7 against 7.5 months) and were still accruing in year two. SLEDAI-2K fell from 13 to 2, anti-dsDNA from 205 to 54 IU/mL, and glucocorticoids were reduced in 72%. In eight paired biopsies, activity fell while chronicity rose.

This is uncontrolled, single-centre and small. Its main lesson is about patience: a deeper response may take more than a year, so judging belimumab at 12 months may be too early.

  • Over half of patients reached protein-to-creatinine ratio under 200 mg/g by two years on add-on belimumab.
  • Deep responses took a median of about 18 months, longer than conventional response.
  • Lower baseline proteinuria was the only consistent predictor of response at 12 months.
  • Consider aiming beyond conventional complete renal response, while recognising its long-term benefit is unproven.
  • Paired biopsies showed falling activity but rising chronicity, so continued kidney protection matters.

Why it matters

Current response definitions may be setting the bar too low for kidneys that need decades of function.

Don't overread it

This uncontrolled, single-centre cohort used a study-specific endpoint not yet linked to long-term kidney outcomes.

The statistics, in plain English

These are available-case percentages in 43 patients, so each patient shifts the rate by over two points. The biopsy findings rest on eight patients.
