Transplant recipients see many clinicians, and a skin lesion is often first noticed by the transplant team or the GP. A risk tier buried in a dermatology letter helps nobody.
State the tier in the first line of every letter — for example, 'High skin cancer risk transplant recipient: review every 3 months; low threshold for biopsy.' It tells the next clinician how seriously to take a new lesion without having to read the history.
- Write the tier and review interval in the first line of the letter.
- Copy the letter to the transplant team.
- Give the patient the same wording so they can ask for an early review.
