Psoriasis was linked to more liver and lung disease in 320,000 patients

A UK phenome-wide study mapped psoriasis against 225 disease groups. Also: a more sensitive pemphigoid serology test, a prognostic index for advanced cutaneous lymphoma, and choosing systemics by comorbidity.

The edition in brief

A UK cohort compared 320,220 people with psoriasis with 1.7 million matched controls across 225 groups of hospital-recorded diagnoses. Beyond the skin, psoriasis was associated with liver disease (HR 1.47), substance misuse including alcohol and tobacco (HR 1.26) and interstitial lung disease (HR 1.33), after adjustment for alcohol, smoking, weight and deprivation; the authors frame these as hypothesis-generating associations. A single-centre laboratory study of 100 bullous pemphigoid sera found an ELISA against the LABD97 fragment of BP180 detected 87% of cases, against 66% for the standard BP180 NC16A test and 52% for BP230, and picked up 83% of sera negative on both. In 277 patients with advanced mycosis fungoides or Sézary syndrome, a four-factor prognostic index separated median progression-free survival from 4.6 years to 0.9 years. A feasibility study of AI-assisted line-field optical coherence tomography found subclinical basal cell carcinomas on normal-looking facial skin in 9.3% of high-risk inpatients. A JAAD review sets out how to choose systemic psoriasis therapy around a patient's inflammatory comorbidities and cancer history.

A LABD97 ELISA caught pemphigoid that standard tests missed

A negative standard pemphigoid ELISA does not rule the disease out; rely on biopsy with direct immunofluorescence when suspicion is high.

A four-factor index sorts risk in advanced mycosis fungoides

In advanced mycosis fungoides or Sézary syndrome, consider scoring the four-factor prognostic index alongside stage when counselling and planning referral.

AI-assisted imaging found hidden basal cell carcinomas on normal-looking faces

Imaging-based screening can find subclinical facial basal cell carcinomas, but it is research-stage, not a reason to change practice.
