Recurrent gestational diabetes linked to about ten times the later diabetes risk
Recurrent GDM carried about a tenfold diabetes risk in a 326,000-mother cohort, highest in Asian women. Also: compounded GLP-1 recalls, missed diagnoses, semaglutide and the kidney, and a weekly insulin–GLP-1 combination.
The edition in brief
A Kaiser Permanente cohort of 326,115 mothers found gestational diabetes was associated with almost six times the risk of later diabetes, and recurrent GDM with almost ten times — about 12 times in Asian women, who also had the highest recurrence rate. The FDA has listed Class II recalls of compounded tirzepatide and semaglutide syringes from one US pharmacy over sterility assurance. A US claims study found one in three adults with laboratory-confirmed diabetes had no diagnosis code, no treatment, or neither a year later, with more comorbidity linked to less action. A prespecified pooled analysis of SELECT, FLOW and SOUL (30,787 participants) found semaglutide lowered a kidney composite by 16%. The pearl covers postpartum glucose testing after GDM. The practice-changer is COMBINE 4: once-weekly IcoSema lowered HbA1c 0.88 points more than daily glargine in insulin-naive adults, with weight loss instead of gain and fewer hypoglycaemic episodes.
Recurrent gestational diabetes and the tenfold later risk
After a second GDM pregnancy, plan regular diabetes screening and prevention as you would for prediabetes.
Compounded tirzepatide and semaglutide syringes recalled over sterility
Ask where each patient's GLP-1 or tirzepatide injection comes from, and move them to licensed product where you can.
One in three lab-confirmed diabetes cases left uncoded or untreated
Make sure every diabetes-range HbA1c ends in a recorded diagnosis and a plan, especially in multimorbid patients.
Semaglutide and kidney outcomes across SELECT, FLOW and SOUL
In patients with CKD or vascular disease, semaglutide's kidney benefit is a reason to choose it, alongside RAS blockade and SGLT2 inhibition.
The postpartum glucose test after GDM
Book the postpartum OGTT before the woman leaves hospital, and keep screening for life.
Weekly IcoSema against daily glargine when starting insulin
When a patient on oral drugs needs insulin, a weekly insulin–GLP-1 combination is worth considering over basal insulin alone where available.
