Transcatheter tricuspid repair cut death or heart failure admission by 60% over three years
TRIC-I-HF randomised 360 older patients with severe tricuspid regurgitation. Also: conduction system pacing after node ablation, ICDs in nonischaemic HFrEF, differences between ARBs, and screening diabetes clinics for hidden heart failure.
The edition in brief
In TRIC-I-HF (NEJM), transcatheter tricuspid repair plus medical therapy beat medical therapy alone in 360 patients with symptomatic severe tricuspid regurgitation, mean age 80: win ratio 2.42 at one year and HR 0.40 for death or heart failure admission through three years. CONDUCT-AF, an 82-patient European trial, found left bundle branch area pacing non-inferior to biventricular pacing for LVEF improvement six months after AV node ablation, with shorter procedures. A meta-analysis of six RCTs found primary-prevention ICDs in nonischaemic HFrEF lowered sudden death (RR 0.41) and cardiovascular death (RR 0.75) but not all-cause death (RR 0.88, CI crossing 1). A network meta-analysis of 27 trials ranked candesartan and telmisartan as the ARBs with MACE reduction, though comparisons were mostly indirect. TARTAN-HF found NT-proBNP screening in high-risk diabetes diagnosed heart failure in one in four, mostly HFpEF. The pearl covers reading NT-proBNP in obesity, AF and CKD.
Conduction system pacing after AV node ablation held up against biventricular pacing
For pace-and-ablate in heart failure with AF, conduction system pacing is a reasonable alternative to biventricular pacing where the expertise exists.
Primary-prevention ICDs in nonischaemic HFrEF: fewer sudden deaths, unclear mortality gain
Individualise primary-prevention ICDs in nonischaemic HFrEF; the reduction in sudden death is clear, the overall survival gain is not.
Not every ARB may be equal for cardiovascular events
When choosing an ARB, consider candesartan or telmisartan, which have outcome-trial support for reducing cardiovascular events.
NT-proBNP screening found heart failure in one in four high-risk diabetes patients
In high-risk patients with diabetes, consider NT-proBNP screening — one in four had unrecognised heart failure.
Reading NT-proBNP in obesity, AF and kidney disease
Read NT-proBNP against weight, rhythm, kidney function and age, and echo when it does not fit the patient.
Transcatheter tricuspid repair in symptomatic severe regurgitation
Refer patients with symptomatic severe tricuspid regurgitation for heart team assessment for transcatheter repair.
