Ask young people directly about suicidal thoughts

Ask young people directly and privately about suicidal thoughts and access to means.

Asking directly about suicidal thoughts does not plant the idea or raise risk; that is well established. Vague questions get vague answers.

See the young person alone for part of the consultation, explain the limits of confidentiality first, and use plain words: are you having thoughts of ending your life?

  • Ask about suicidal thoughts in plain words, not euphemisms.
  • See adolescents alone for part of the visit, after explaining confidentiality and its limits.
  • Ask about access to medicines, pesticides and other means at home.
  • Record what was asked and the plan agreed.

Why it matters

The question most clinicians hesitate to ask is the one that opens the conversation.
