Acetic acid chromoendoscopy doubled suspicious lesions in Barrett's surveillance but found no more neoplasia
The ACID trial in Dutch community endoscopy, an oral IL-23 receptor blocker in ulcerative colitis, STRIDE in poor-prognosis liver cancer, and an FDA supplement for an adalimumab biosimilar.
The edition in brief
The ACID trial, a stepped-wedge cluster randomised trial of 2,267 people under Barrett's surveillance at 18 Dutch community hospitals, found acetic acid chromoendoscopy flagged suspected lesions in 14% against 6% but did not raise neoplasia detection (7.4% vs 7.1%, OR 0.97). A phase 2b trial of oral icotrokinra, a peptide blocking the IL-23 receptor, in 252 adults with moderate to severe ulcerative colitis found week-12 clinical response of 55 to 63% across doses against 27% on placebo, and clinical remission of 30% at 400 mg against 11%. SIERRA, a single-arm phase 3b study of tremelimumab plus durvalumab in 111 patients with unresectable hepatocellular carcinoma and Child-Pugh B7 to B8 function, performance status 2 or main portal vein thrombosis, found grade 3 or 4 treatment-related adverse events in 20% and objective response in 14%. The FDA recorded a supplement on 25 September for the adalimumab biosimilar adalimumab-aqvh (Yusimry); the record does not describe the change. The pearl covers the Seattle biopsy protocol.
Oral icotrokinra roughly doubled clinical response over placebo in ulcerative colitis
Oral IL-23 receptor blockade is a promising route for ulcerative colitis, but this is phase 2 evidence; the drug is not approved for this use.
STRIDE in poor-prognosis liver cancer: 20% serious treatment-related toxicity and 14% response
Tremelimumab plus durvalumab can be considered for selected patients with unresectable hepatocellular carcinoma and Child-Pugh B7 to B8 function or performance status 2, with modest expectations of response.
FDA records a supplement for the adalimumab biosimilar adalimumab-aqvh
No change to adalimumab prescribing in IBD follows from this record.
Follow the Seattle protocol in Barrett's surveillance
Take four-quadrant biopsies every 2 cm of Barrett's segment, plus targeted biopsies of any visible lesion, after careful inspection.
Acetic acid chromoendoscopy did not improve neoplasia detection in community Barrett's surveillance
In routine Barrett's surveillance, acetic acid spray adds biopsies without adding diagnoses; careful white-light inspection with the Seattle protocol remains the standard.
