Chemical eye injury: irrigate first, examine second

Irrigate a chemical eye injury immediately and thoroughly, remove retained particles, and grade it only after the pH is neutral.

In any chemical or thermochemical eye injury, start copious irrigation immediately, before testing acuity or taking a full history. Continue until the conjunctival pH is neutral, evert the lids and sweep the fornices to remove retained particles, which is particularly important with calcium carbide and lime. Only then grade the burn by limbal and conjunctival involvement and decide on referral.

  • Begin irrigation at once with whatever clean fluid is available.
  • Check pH and continue until it is neutral.
  • Evert the lids and sweep the fornices for retained particles.
  • Grade limbal involvement after irrigation and refer severe burns urgently.

Why it matters

Minutes of delay before irrigation can decide whether the limbus survives.
