After endoscopic resection of a pT1 colorectal cancer, the decision about further surgery rests on the pathology report. The established high-risk features to state explicitly are lymphovascular invasion, poor differentiation, tumour budding grade, depth of submucosal invasion, and the status of the deep and lateral margins, with the distance to the deep margin where measurable.
A report that leaves any of these out forces the multidisciplinary team to guess or to re-request the slides.
- State lymphovascular invasion as present or absent.
- Give the grade of differentiation.
- Report tumour budding grade.
- Record depth of submucosal invasion.
- Give deep and lateral margin status and the deep margin distance.
Why it matters
The surgeon's decision is only as complete as the report.
