- Design
- Prospective population pharmacokinetic study
- Population
- 15 term neonates with hypoxic-ischaemic encephalopathy on therapeutic hypothermia
- Primary outcome
- Levetiracetam pharmacokinetic parameters and covariates
- Effect
- Clearance 4.27 L/h, rising with creatinine clearance and serum sodium; no exposure–response relationship
LEVNEONAT-1 measured levetiracetam concentrations in 15 term neonates with hypoxic-ischaemic encephalopathy and seizures during therapeutic hypothermia. They received a loading dose of 30–60 mg/kg, then a quarter of the loading dose every eight hours. A population pharmacokinetic model was built from five samples per baby.
A two-compartment model scaled by weight fitted the data, with typical clearance of 4.27 L/h. Clearance rose with creatinine clearance and with serum sodium; the sodium relationship has not been described before and may reflect encephalopathy severity. Neither peak concentration nor early exposure was related to the reduction in seizures.
With 15 babies, this is a modelling study, not a dosing trial. It does show that kidney function in cooled, asphyxiated neonates — which often changes over the first days — should be considered when maintaining levetiracetam.
- Consider renal function when continuing levetiracetam in neonates with hypoxic-ischaemic encephalopathy.
- Expect clearance to change as kidney function recovers after asphyxia.
- There was no exposure-response link here, so higher loading doses are not supported by these data.
- Levetiracetam is not the first-line drug for neonatal seizures in many guidelines; follow your unit protocol.
Why it matters
Neonatal levetiracetam dosing is often extrapolated; these data show what actually drives exposure in cooled babies.
Don't overread it
Fifteen neonates are too few to judge efficacy or set a dosing regimen.
The statistics, in plain English
Population pharmacokinetic models estimate a typical value and how much babies vary around it. Inter-individual variability of 15% for clearance is modest; 98% for peripheral volume means that parameter is poorly pinned down.
