PCSK9 inhibitors: a 700-outcome signal scan found little to worry about

Reassure patients starting a PCSK9 inhibitor that a large real-world scan found no new serious safety signals.

Design
Sequence symmetry analysis with active comparator, plus disproportionality analysis
Population
36,163 PCSK9 inhibitor and 34,269 ezetimibe initiators in France, 2018–2023
Primary outcome
Potential safety signals across 700 outcomes
Effect
2 potential signals (inhaled steroids SRa 1.19; colony-stimulating factors SRa 2.25), both likely confounded

Real-world safety data for PCSK9 inhibitors remain limited compared with statins. This French study used the national health insurance databases to run a sequence symmetry analysis — looking at whether new events or new prescriptions cluster after, rather than before, starting a drug — in 36,163 people starting a PCSK9 inhibitor between 2018 and 2023. Ezetimibe starters (34,269) were used as an active comparator to remove trends and indication bias. Designated medical events were then checked in the WHO VigiBase database.

Across 700 outcomes, only two potential signals emerged: new inhaled corticosteroid prescriptions (adjusted sequence ratio 1.19, 95% CI 1.06–1.35) and new colony-stimulating factor prescriptions (2.25, 95% CI 1.37–4.17). Neither was supported in VigiBase, and the authors judged both likely to reflect residual confounding.

For prescribers, this is broadly reassuring alongside the trial safety data. Injection-site reactions and nasopharyngitis-type symptoms remain the familiar adverse effects; the scan did not surface new serious harms.

  • No new serious safety signal emerged for PCSK9 inhibitors across 700 outcomes.
  • Respiratory symptoms after starting may prompt inhaled steroid use; the signal was likely confounded.
  • Continue to report suspected adverse reactions, as long-term real-world data are still building.
  • Counsel patients on injection-site reactions, the most familiar adverse effect.

Why it matters

Hypothesis-free signal scans are how unexpected harms are found, and this one found none of note.

Don't overread it

Sequence symmetry analysis detects signals; it cannot rule out rare or slowly developing harms.

The statistics, in plain English

A sequence ratio above 1 means an event was more often started after the drug than before it. Adjusting against ezetimibe removes general trends. With 700 outcomes tested, a couple of positive results would be expected by chance alone.
