The Fleischner Society guidance for pulmonary nodules applies to nodules found incidentally on CT in adults aged 35 or over. It is not intended for lung cancer screening, where Lung-RADS or the screening programme's own protocol applies, nor for patients with known cancer or immunosuppression, in whom a nodule is managed in the context of the underlying disease. Using the wrong framework produces the wrong follow-up interval, often more scans than needed.
- Apply Fleischner guidance only to incidental nodules in adults aged 35 or over.
- Use Lung-RADS or the programme protocol for nodules found on screening CT.
- Manage nodules in patients with known cancer or immunosuppression clinically, not by Fleischner intervals.
- State in the report which framework the recommendation follows.
Why it matters
Applying the wrong nodule guideline is a common source of unnecessary or missed follow-up.
