- Design
- Prospective single-centre observational registry cohort
- Population
- 176 adults with systemic lupus erythematosus starting rituximab or belimumab
- Primary outcome
- Achievement of low disease activity (SLEDAI-2K ≤4, prednisolone ≤5 mg)
- Effect
- HR 0.92 (0.57 to 1.48); overlap-weighted HR 0.86 (0.54 to 1.37)
A prospective single-centre registry cohort at a UK hospital compared 175 rituximab courses in 148 patients with 58 belimumab courses in 58 patients with systemic lupus erythematosus, from 2011 to 2025.
Belimumab users had higher baseline activity. After adjustment, achieving low disease activity (SLEDAI-2K 4 or less with prednisolone 5 mg or less) did not differ (HR 0.92, 95% CI 0.57 to 1.48; overlap-weighted HR 0.86, 0.54 to 1.37). Drug survival was similar first-line; as a second-line biologic, belimumab persisted longer (HR 1.34, 1.03 to 1.77).
With small numbers and non-random treatment choice, this does not settle which to use. It offers some reassurance that the choice can rest on phenotype, access and cost.
- Choose between rituximab and belimumab on lupus phenotype, prior treatment, access and cost.
- Set a target of low disease activity with prednisolone 5 mg or less when starting either biologic.
- Consider belimumab as a second-line biologic where persistence matters, noting the observational evidence.
- Review biologic response at defined intervals rather than continuing indefinitely.
Why it matters
Head-to-head evidence between the two is scarce, and cost differences are large in Indian practice.
Don't overread it
A small, single-centre observational comparison; confounding by indication cannot be excluded.
The statistics, in plain English
Hazard ratios near 1 with wide intervals crossing 1 mean no detectable difference, but the study was too small to rule out a meaningful one.
