Hypertension in pregnancy has climbed sharply, and GLP-1 exposure is reaching the booking clinic
US rates of hypertensive disorders of pregnancy rose by about three-quarters in eight years. Alongside: what is and is not known after inadvertent GLP-1 receptor agonist exposure, barbed uterine closure, and epidural hydromorphone at caesarean.
The edition in brief
A cross-sectional analysis of more than 33 million US live births found age-standardised rates of hypertensive disorders of pregnancy rose from 62.7 to 109.1 per 1,000 between 2016 and 2024, with a steeper climb from 2021 and increases in every racial and ethnic group. A Cochrane review found only one randomised trial comparing gestational diabetes screening in maternity versus general settings: low-certainty evidence that maternity settings improve compliance and speed access to care, and no usable evidence on clinical outcomes. A single-centre Italian matched cohort found no clear difference in time to conception or later obstetric outcomes after barbed versus monofilament uterine closure, but estimates were imprecise. A 96-woman Chinese trial linked 0.3 mg epidural hydromorphone added to ropivacaine with less nausea and vomiting during caesarean, with modest pain differences partly based on post-registration choices. A BJOG narrative review found no clear safety signal after inadvertent periconception GLP-1 receptor agonist exposure, but too little data to exclude harm; the practical step is to ask about these drugs at booking and counsel individually.
US hypertensive disorders of pregnancy rose 74% from 2016 to 2024
Treat hypertension in pregnancy as a routine, high-volume problem: plan postpartum blood pressure checks and long-term risk counselling for every affected woman.
Where to screen for gestational diabetes: one trial, little certainty
Screening in maternity settings may improve completion and speed access to diabetes care, but there is no reliable evidence on clinical outcomes.
Barbed versus monofilament uterine closure: no clear fertility difference
Barbed closure was not clearly linked to slower conception or worse later pregnancy outcomes, but the study was too small to establish safety.
Epidural hydromorphone at caesarean: less nausea during traction
Adding 0.3 mg epidural hydromorphone was associated with markedly less intraoperative nausea; the pain benefit was small and needs confirmation.
After a hypertensive pregnancy, the risk does not end at discharge
Plan an early postpartum blood pressure check and give clear warning symptoms to every woman with a hypertensive pregnancy.
Ask about GLP-1 receptor agonists at every booking visit
Ask about GLP-1 receptor agonist use at booking, and counsel exposed women that evidence so far is reassuring but insufficient to exclude harm.
