Preoperative radiotherapy for pancreatic cancer was linked to clearer margins but not longer survival
A matched cohort of 452 resected patients found clearer margins without longer life. Also: women's higher wound and recurrence risk after ventral hernia repair, gastrojejunostomy configuration after Whipple, and parathyroid imaging in thyroidectomy.
The edition in brief
A propensity-matched cohort from two high-volume centres compared neoadjuvant chemotherapy with or without radiotherapy before pancreatic cancer resection. Radiotherapy was associated with more R0 resections (85.0% vs 52.2%) and node-negative disease, but not with better event-free survival (HR 1.10) or overall survival (HR 1.02), and distant recurrence dominated in both groups. A registry study of 31,358 ventral hernia repairs found women had more complex hernias, higher 30-day wound infection and readmission, and higher recurrence at two and three years, while reporting greater improvement in quality of life. The IPAD randomised trial of 158 patients found no difference in delayed gastric emptying between side-to-side and end-to-side gastrojejunostomy after pancreatoduodenectomy. A single-centre study developed and validated a three-item endoscopic classification of fundoplication integrity that predicted pathological reflux with an AUC of about 0.87. The European Society of Endocrine Surgeons issued recommendations on near-infrared autofluorescence, with or without indocyanine green, to identify parathyroid glands, based on a meta-analysis of 34 studies and 9,322 patients.
Neoadjuvant radiotherapy in pancreatic cancer: better margins, same survival
Adding radiotherapy to neoadjuvant chemotherapy improved pathological margins but was not associated with longer survival; do not add it for margins alone.
Women had more wound complications and late recurrence after ventral hernia repair
When consenting women for incisional hernia repair, discuss higher wound and late recurrence risk alongside the larger expected benefit.
Gastrojejunostomy configuration did not change delayed gastric emptying after Whipple
Side-to-side and end-to-side gastrojejunostomy gave similar delayed gastric emptying rates; choose by surgeon familiarity.
Three endoscopic signs identify a failing fundoplication
After antireflux surgery, note valve position, disruption and visible hiatus at endoscopy; two or more signs make true reflux likely.
Check calcium early after total thyroidectomy
Check postoperative calcium or PTH after total thyroidectomy and teach patients the early symptoms of hypocalcaemia.
European recommendations on autofluorescence to protect parathyroids
Where available, consider autofluorescence imaging during total thyroidectomy to help identify and preserve parathyroid glands.
